PCT-agreed and established (written) arrangements to ensure transition of care for service users between adult mental health services and mental health services for older people.

Rationale
Continuity of care is recognised as critical to ensuring mental health needs are being met, and there is evidence that the links for patients between working age adult services and services for older people are not good. The MHNSF and the OPNSF are committed to a patient journey whole systems approach which will ensure a smooth transition from one service to the other.
