Rationale
Standard 4 of the MH NSF states that all mental health users on the Care Programme Approach (CPA) should have a copy of a written care plan which includes the action to be taken in a crisis by service users, their carers and their care coordinators, which advises the GP how they should respond if the service user needs additional help and which is regularly reviewed by the care coordinator. It also states that all patients should have a copy of a written care plan agreed on discharge from inpatient care. Patients having copies of their care plan is evidence of CPA Systems in place and working effectively.
