Emergency readmissions to hospital within 28 days of discharge following a stroke, as a percentage of live stroke discharges (age and sex standardised).
Rationale
Analyses have shown that around 5% of patients discharged from NHS hospitals are readmitted as an emergency within 28 days. For older people the figure is about 7%. Readmissions may be as a result of poor treatment in hospital, or poor or badly organised rehabilitation and support services when a person is transferred home following treatment. Although it is difficult to tell how many readmissions are linked to the original treatment, readmissions can be avoided if the correct local procedures are in place. The NHS may be helped to prevent potentially avoidable readmissions by seeing comparative figures and learning lessons from the experience of hospitals with low readmission rates.
